What to Eat on GLP-1 (Semaglutide, Tirzepatide) — Lancaster PA Guide

When you're on Semaglutide or Tirzepatide, your appetite tanks. You're not hungry. Meals feel like homework.

The temptation: eat as little as possible. The result: rapid weight loss — but mostly muscle, not fat.

Here's how to eat on GLP-1 so you lose fat, not muscle. Written for Alesca patients in Lancaster, PA by Morgan White, MSN.

The 3 rules

  1. Hit your protein target daily — even when you don't feel like eating
  2. Hydrate aggressively — 80-100 oz water minimum
  3. Skip greasy, spicy, and huge meals — they'll make you feel awful

Everything else is optional. These three are non-negotiable.

Rule 1: Protein target

Formula: 0.7 to 1.0 g protein per pound of goal body weight (not current weight — GOAL weight).

If your goal weight is 150 lbs: 105-150 g protein daily.

Why this matters: Rapid weight loss without adequate protein = muscle loss. Muscle burns calories at rest. Less muscle = slower metabolism = weight regain when you stop GLP-1.

Meal-friendly targets:

  • Breakfast: 25-30 g (Greek yogurt + protein powder + fruit)
  • Lunch: 30-40 g (chicken salad, tuna wrap, protein shake)
  • Dinner: 30-40 g (salmon, chicken, ground beef)
  • Snacks: 10-20 g each (cheese stick, protein bar, jerky)

Protein shakes are your friend on GLP-1. When you can't eat a meal, drink it. 20-30 g protein in 250 calories, digests easily.

Rule 2: Hydration

Target: 80-100 oz water daily minimum. 100+ if you exercise.

Why this matters: GLP-1 medications slow gastric emptying. You feel full longer, drink less, get dehydrated fast. Dehydration causes:

  • Fatigue (blamed on the med, actually dehydration)
  • Constipation (worst GI side effect long-term)
  • Headaches
  • Dizziness on standing
  • Kidney stress

Tricks:

  • Fill a 40 oz bottle at start of day. Refill by 2pm. Finish by 8pm.
  • Add electrolytes (LMNT, Nuun, or plain salt in a pinch)
  • Sparkling water counts — but skip if it makes you nauseous
  • Herbal tea counts
  • Coffee mildly diuretic — doesn't fully count toward water
  • Alcohol NEGATIVE — reduces hydration

Sign you're hydrated: Pale yellow urine. Dark yellow = drink more.

Rule 3: Foods to avoid (make you feel awful)

Fried food: Fried chicken, French fries, tempura, chicken tenders. Fat digests slowly + GLP-1 slows digestion further = nausea for hours.

Greasy meat: Bacon, sausage, high-fat ground beef, ribs. Same issue.

Spicy food: Buffalo wings, spicy curry, hot peppers. On GLP-1, they hit twice as hard.

Large portions: Even healthy food. Overfill your stomach and you WILL vomit. Small meals only.

Carbonated drinks: Bloating + fullness discomfort.

Alcohol: Reduces hydration, worsens nausea, empty calories, and slows fat loss.

Ultra-processed carbs on empty stomach: Sugary cereal, donuts, bagels. Spike + crash = nausea + fatigue.

Foods that work best on GLP-1

Best: Grilled/baked lean protein + steamed veggies + small serving of complex carb.

Meal examples:

Breakfast: - Greek yogurt + berries + 1 tbsp nuts (25 g protein) - 2 eggs + spinach + slice of whole-grain toast (15-20 g protein) - Protein smoothie: 1 scoop whey + 1/2 banana + spinach + almond milk (25 g protein) - Cottage cheese + peaches + 1 tbsp chia seeds (25 g protein)

Lunch: - Grilled chicken salad with olive oil dressing (35 g protein) - Turkey wrap on whole-grain tortilla + apple (30 g protein) - Salmon + roasted veggies + small quinoa portion (30 g protein) - Egg salad on 2 slices whole-grain (20 g protein)

Dinner: - Baked chicken breast + roasted broccoli + sweet potato (35 g protein) - Salmon + asparagus + wild rice (30 g protein) - Ground turkey stir-fry with veggies + brown rice (35 g protein) - Shrimp tacos on corn tortillas with cabbage slaw (30 g protein)

Snacks: - String cheese + apple (10 g protein) - Protein bar (aim 15+ g protein, <20 g carbs) — Quest, Barebells, or Ratio - Beef jerky (10-15 g protein per oz) - Greek yogurt cup (20 g protein) - Handful of almonds + a hard-boiled egg (12 g protein)

Meal timing

GLP-1 slows digestion. You'll feel full longer than before.

  • Space meals 4-6 hours apart — you probably won't be hungry sooner
  • Small snacks if you MUST eat between — protein-based
  • Stop eating 2-3 hours before bed — sleep on empty stomach reduces reflux

Don't force meals if you're not hungry. Miss lunch occasionally? Fine — as long as protein target is hit across the day (front-load breakfast + heavier dinner).

Special situations

When nausea strikes

Follow the BRAT diet temporarily: - Bananas - Rice (white, small portion) - Applesauce - Toast (dry, whole grain)

Add: - Bone broth (electrolytes + protein) - Plain crackers - Ginger tea (fresh ginger works better than teabag)

Avoid until nausea resolves: - Dairy (harder to digest) - Any raw vegetables - Anything fried or greasy - Coffee (irritates stomach)

When constipation kicks in

Standard on GLP-1 by week 3-4.

  • Fiber: 25-35 g daily. Focus on soluble fiber (oats, chia seeds, apples).
  • Magnesium citrate: 200-400 mg at bedtime
  • Miralax: PEG 3350, safe daily use
  • Prunes or prune juice: works fast for many
  • Water: increase to 100+ oz
  • Walking: 20-30 min daily improves GI motility
  • Coffee: often triggers a bowel movement 30-60 min after

When you're stuck at a plateau

Common at month 3-4. Not always about the meds.

  • Assess protein intake — often too low, causing muscle loss + slowed metabolism
  • Track hydration — dehydration = false plateau
  • Sleep quality — poor sleep = higher cortisol = weight retention
  • Stress level — chronic stress = higher cortisol = same problem

Supplements to consider

Not required, but helpful:

  • B12 shots monthly — most GLP-1 patients benefit
  • Vitamin D if levels low (test first)
  • Magnesium citrate for constipation + sleep support
  • Fish oil / omega-3 — anti-inflammatory
  • Electrolyte powder (LMNT, Element) — replaces salt lost with fluid loss
  • Whey protein or plant protein — when meals are hard

Morning (7 am): - Greek yogurt (1 cup) + 1 scoop whey (25 g protein each) + 1/2 cup berries + 1 tbsp almonds - Coffee with splash of milk - 40 oz water - Total: 50 g protein, ~450 cal

Late morning (10 am): - 1 string cheese + 1 apple - 20 oz water - 10 g protein, ~150 cal

Lunch (1 pm): - 4 oz grilled chicken breast + big salad + olive oil vinaigrette + 1/4 avocado - 20 oz water - 35 g protein, ~450 cal

Afternoon (4 pm): - 1 hard-boiled egg + handful almonds - Herbal tea - 10 g protein, ~200 cal

Dinner (7 pm): - 4 oz salmon + roasted broccoli + 1/2 cup wild rice - 20 oz water - 30 g protein, ~500 cal

Daily total: 135 g protein, ~1,750 calories, 100 oz fluid
