Masseter Botox is one of the most requested treatments at Alesca Aesthetics in Lancaster, PA — and one of the least understood.
It does two things at once: cosmetically slims a wide, square lower face into a softer V-shape, and medically stops teeth grinding, jaw clenching, and TMJ pain. Same injection. Two wins.
Here's exactly how it works and whether it's right for you.
What the masseter is
The masseter is your primary chewing muscle — the thick band that runs from your cheekbone down to your jaw angle. If you clench your teeth, you'll feel it bulge under your fingers along your jawline.
For some people, the masseter is naturally overdeveloped (genetics, chronic grinding, high stress). This creates:
- Square, wide lower face
- Prominent jaw angles
- Chronic jaw tension
- Teeth grinding (bruxism)
- Headaches, especially morning
- TMJ pain and clicking
Masseter Botox weakens the muscle. Over weeks, the muscle atrophies (safely), your face softens, and grinding/clenching stops.
Who's a candidate
Cosmetic candidates — want jaw slimming: - Square-shaped lower face you'd prefer softer - Prominent jaw muscles visible when relaxed - Asymmetric masseters (one side more developed) - Younger clients wanting more feminine facial shape
Medical/functional candidates — need relief: - Chronic teeth grinding (bruxism) - Wake up with sore jaw or headache - Diagnosed TMJ dysfunction - Cracking teeth or breaking dental work from clenching - Chronic tension headaches originating in jaw
Best candidate: BOTH — you want cosmetic slimming AND grinding relief. Same treatment, both wins.
The results — timeline
Weeks 1-2: Grinding reduces almost immediately. Jaw feels looser.
Weeks 2-4: Chewing endurance slightly reduced (a big steak takes longer — normal). No visible slimming yet.
Weeks 4-8: Muscle starts to atrophy from disuse. Jaw angle softens.
Months 2-3: Peak cosmetic result. Face notably slimmer, jaw angle softer.
Months 4-6: Effect maintains. Some clients note grinding starting to return around month 4-5.
Rebook: Every 4-6 months for cosmetic maintenance. Every 3-4 months if aggressive grinder needing symptom control.
Cost — masseter Botox pricing in Lancaster PA
Same $13/unit as regular Botox at Alesca. Dosing per side:
| Purpose | Units per side | Total units | Cost |
|---|---|---|---|
| Mild TMJ relief only | 15-20 | 30-40 | $390-$520 |
| Moderate slimming + grinding | 20-30 | 40-60 | $520-$780 |
| Significant slimming (strong masseters) | 30-40 | 60-80 | $780-$1,040 |
| Very developed masseter (max dose) | 40-50 | 80-100 | $1,040-$1,300 |

What NOT to expect
Not permanent: Effect lasts 4-6 months. Muscle grows back with re-use unless you rebook.
Not instant: Grinding relief comes fast, but cosmetic slimming takes 4-8 weeks minimum.
Not painless when chewing tough food for first few weeks: hard bagels, tough steak, and jerky may feel strange. Chewing endurance returns as your other muscles compensate.
Not a fix for facial fat: If your wide face is from cheek fat, not muscle, masseter Botox does nothing. Consult can distinguish.
Not for everyone: If you have weak masseter to begin with, further weakening could cause chewing difficulty. Consult required to assess baseline.
The consultation matters
Morgan will:
- Palpate your masseter while you clench and relax — assess baseline muscle mass
- Distinguish muscle vs fat contribution to your face shape
- Mark injection points — usually 3 per side, in a triangle pattern
- Discuss dose based on strength of muscle + your goals
- Screen for contraindications — myasthenia gravis, prior negative Botox reactions, active infections at injection site
What the treatment feels like
30-45 minutes total. Injection itself takes 5 minutes.
- Ice or numbing cream applied 10 min before (optional)
- 3 injections per side — feels like a firm pinch each
- Some clients feel deep pressure during injection — brief
- Zero downtime — drive yourself home, back to work
Post-treatment: - Ice if desired for 10 min - Same Botox aftercare rules — no lying flat 4h, no rubbing, no exercise 24h - Read full Botox aftercare guide
Side effects (uncommon)
- Chewing fatigue: Normal for first 2-4 weeks. Resolves as other muscles compensate.
- Asymmetric smile: Very rare — occurs if Botox migrates to risorius (smile muscle). Usually resolves within 4-6 weeks.
- Facial hollowing (long-term overuse): Aggressive dosing every 3 months for years can cause visible cheek hollowing. We don't dose this way.
- Injection site bruising: 5-10% of clients. Resolves in 3-7 days.
Red flags — call us: - Difficulty swallowing (very rare but requires evaluation) - Persistent asymmetry beyond 6 weeks - Progressive weakness on one side
Combining with other treatments
Masseter + Botox for upper face: Very common — treat forehead, glabella, crow's feet + masseter same appointment. Total budget: $650-$1,500 depending on units.
Masseter + cheek filler: For clients wanting both jaw slimming AND midface lift. Effect compounds — slimmer + more contoured.
Masseter + jawline filler: For clients whose square jaw comes from bone structure + muscle. Muscle Botox alone doesn't fully address. Filler contours; Botox slims. Discussed at consult.
Common questions
Will this make my face look weird? No. Cosmetic slimming is gradual and natural-looking. Aggressive over-treatment can look gaunt — Morgan doses conservatively.
Can I still eat hard food? Yes. Bite force reduced ~20-30%. Most clients don't notice except with jerky or very tough steak. Full endurance returns in 4-6 weeks.
Does insurance cover masseter Botox for TMJ? Sometimes. Depends on plan. Ask us for a letter of medical necessity + you submit to insurance. Alesca is not in-network — patient responsible for insurance interaction.
Will grinding come back after Botox wears off? Yes, unless treated with a nightguard or stress reduction alongside. Masseter Botox is symptom control, not cure.
Can I combine with a nightguard? Yes — best of both. Nightguard protects teeth; Botox stops clenching force at the muscle level.
How is this different from filler for jawline? Filler ADDS contour. Botox REMOVES contour. Different tools, different goals. Some clients need both.
