Your lip filler appointment at Alesca in Lancaster, PA is done. Now the anxiety phase begins — "why are they so big?", "why are they uneven?", "did I make a mistake?"
Every panic is normal. Here's the exact day-by-day timeline of what to expect + the aftercare protocol that maximizes your results.
The 30-second aftercare summary
Hours 0-2: Ice on and off. No touching. Nothing on lips.
Hours 2-24: Ice as needed. Avoid: exercise, alcohol, hot food, kissing, straws, spicy food.
Days 1-3: Peak swelling. Keep head elevated when sleeping. Continue avoiding above.
Days 4-7: Swelling resolves. Lips settle into final shape.
Day 14: Follow-up appointment. Assess results, minor touch-up if needed.
Day-by-day expectations
Day of treatment (day 0)
Hour 0: You leave the office. Lips already look noticeably fuller. Some tenderness. May have redness at injection sites.
Hours 1-4: Peak initial swelling. Lips look "big" — larger than the result you asked for. Panic normal. This is water swelling from the trauma, not filler volume.
Hours 4-8: Swelling continues. Slight bruising may appear at injection sites — small purple dots.
Bedtime: Sleep propped up on 2 pillows. Avoid sleeping face-down.
Day 1
Wake up: Lips look BIGGER than yesterday. Also normal. Swelling peaks around 24-36 hours after injection.
What to do: - Ice on/off (10 min on, 10 min off) — anti-inflammatory - Drink lots of water (hydration reduces swelling) - Skip: gym, alcohol, hot food, straws, kissing, aggressive lip movement - Continue elevated head sleeping
What to avoid: - Lip balm or products (yes even Chapstick — for 24 hours minimum) - Makeup on lips - Hot baths, saunas, steam rooms - Blood thinners (ibuprofen, aspirin) — Tylenol only if you need pain relief
Day 2
Swelling still present. Bruising may look worse before better (it always does). Lips may feel firm or "lumpy" — normal.
Common thought at day 2: "They look uneven / weird / wrong."
Response: WAIT. Day 2-3 asymmetry is swelling asymmetry, not filler asymmetry. Do NOT judge results before day 7.
Continue: - Ice as needed - Hydration - Gentle food (soft, room temp, no spicy) - Sleep elevated
Day 3
Swelling starts resolving. Bruises may darken then lighten.
Can resume: - Light exercise (walks, yoga — no heavy lifting or hot yoga) - Lip balm (unfragranced) - Regular skincare - Gentle kissing (partner should know)
Still avoid: - Aggressive lip movement (no dental cleanings, big sandwich bites) - Hot drinks (lukewarm coffee is fine, boiling hot tea is not) - Alcohol (still slight bruising risk)
Day 4-6
Swelling mostly gone. Bruises fading. Lips look like their intended shape more each day.
Resume normal: - Full workouts - Alcohol - Regular eating - Makeup on lips - Kissing (aggressively — go wild)
Watch for: - Any lumps that don't smooth with gentle massage (rare — massage 30 sec twice daily can help) - Any spot that feels tender when others are not (possible small hematoma — usually resolves)
Day 7
Final shape emerging. Any residual swelling gone. What you see now is close to your permanent look.
If something feels off — schedule your 2-week follow-up.
Day 14 — follow-up appointment
Free follow-up included in your $700 flat rate. Morgan assesses:
- Even distribution
- Any lumps needing dissolution
- Whether a touch-up (0.3-0.5 mL) would improve balance
- Long-term satisfaction check
Most clients don't need touch-ups. Some benefit from a small amount added — often free if under 0.3 mL.
Weeks 3-4
Peak final look. Filler fully integrated. This is what you'll live with for 6-18 months.
When to worry — red flags
Call Alesca immediately if you experience:
- Severe pain that worsens over 24-48 hours (rare — could indicate vascular occlusion)
- Blanching (white spots) or dusky purple color that doesn't resolve
- Loss of feeling in areas beyond expected numbness
- Fever, spreading redness, warmth expanding from lips
- Blistering
- Vision changes
These are rare but serious. Call 717-584-4912 immediately. Vascular occlusion (rare) is time-sensitive and requires dissolver injection within hours.
Swelling reduction tips
What works: - Cold compresses (ice pack wrapped in cloth, 10 min on / 10 min off) - Head elevation while sleeping - Hydration (aim for 80+ oz water day 1-3) - Antioxidant-rich diet (berries, dark leafy greens) - Rest
What doesn't work (skip these): - "Anti-swelling" facial exercises — irritates injection sites - Massaging aggressively — pushes filler around unpredictably - Ibuprofen — actually thins blood and INCREASES bruising
Bruising reduction
If you tend to bruise, help it heal:
- Arnica cream/gel — apply 2-3x daily starting day 1
- Arnica tablets — oral supplement, take days 1-5
- Vitamin K cream — applied topically
- Warm compress starting day 3 (not day 1) — helps break down old blood
- Cool cucumber slices — cool + gentle anti-inflammatory
Book with strategic timing: If you bruise easily, plan lip filler at least 2.5-3 weeks before major events.
